CAMP FORMS
- Medical History & Authorized Pickup Forms *Must be completed and turned in prior to attending camp.
- Special Needs Assessment Form
- Medical Paperwork *Completed as directed by camp nurse
The Manalapan Summer Rec Camp is open to resident children currently in grades K-7.
*Campers must be 5 years of age by October 1st of their kindergarten year to register for kindergarten and 6 years of age by October 1st of their first grade year to register for first grade.
